The Policyholder is obliged to notify the Insurer in writing about the inpatient treatment of the Insured Person within three days from the beginning of hospitalization. In case of hospitalization of the Insured Person, the Insured must submit the following documents to PJSC GRAWE UKRAINE Life Insurance:

• a written application for insurance payment;

• copies of the passport and identification code of the Insured Person;

• copies of all medical documents that would reflect the course of treatment (medical reports, medical histories, discharges, etc);

• other documents related to the Insured event (at the request of the Insurer).
