The Policyholder is obliged to notify the Insurer in writing about the surgical operation of the Insured Person. In case of surgical operation of the Insured Person, the Insured must submit the following documents to PJSC GRAWE UKRAINE Life Insurance:

• a written application for insurance payment;

• copies of the passport and identification code of the Insured Person;

• copies of all medical documents from doctors and institutions that performed surgery on the Insured person;

• other documents related to the Insured event (at the request of the Insurer).
